The health Ministry explained to the Ukrainians as to adapt to innovations associated with medical reform.
2018 Ukraine will start healthcare reform. Changes will affect the first primary medicine – the Ukrainians will have to sign with doctors contracts on health care services – Declaration. The Ministry of health said that patients to do with the New year and how to choose a family doctor.
Treatment according to the Declaration
Ukrainians are no longer tied to the doctor’s place of residence. The patient is now able to choose that doctor who works closer to or trusted more. To confirm your selection, you will have to make a Declaration of choice of doctor in primary care. The Declaration is the document that confirms the patient’s choice of doctor. This agreement signed in two copies – one retained by the patient, the other is in the hospital.
The Declaration specifies the data about the patient and about the person you want to alert if the patient’s health will worsen. Also in the document the doctor has to write your details, including business address and contact information so that you can make an appointment. All the information in the Declaration made by the doctor or nurse.
Such a Declaration you need to sign up with your family doctor or therapist. Children will also need a Declaration with the pediatrician – it must be signed by the parents. This agreement is necessary in order that the Ministry of health knew how many patients the doctor accepts and how much you need to pay for it. So, for each patient, which the doctor will sign a Declaration, the hospital will obtain 370 hryvnias per year, for children and pensioners – twice. A doctor will be able to sign the 2000 returns.
Then the data of the patients, the doctor will make the e-health system E-Health. “It will be a registry of patients, physicians and medical institutions. Eventually we will switch to electronic prescriptions, electronic medical records,” explains the acting Minister of health Suprun. The Ministry of health had already presented a demo version of the electronic system ehealth-ukraine.org.
How to choose a doctor
The health Ministry advised to choose the doctor, whom the patient knows who was treated early. If it’s not, you can ask the relatives.
“You can also contact the local health Department. It will tell you which doctors in your locality or area. Another option – just contact any health facility and meet the doctors. Information about them can be obtained from the local health Department or directly to the Registrar’s office”, – it is recommended the Agency.
The family doctor and the Secretary of the Public Council under the Ministry of health Eugene Gonchar advises to check the doctor on several items before you sign with him the Declaration.
The simplest way is reviews and ratings, you can find them on the Internet. The second understanding. The doctor must inspire confidence and speak easy. Third point and most important is competence. Because the patient is not able to objectively assess the professionalism of the doctor, Eugene Gonchar on the page on Facebook advises to ask your family doctor or the therapist a few questions:
“It is clear that the physician must be a supporter of evidence-based medicine, as everything else is shamanism, not medicine. In order to verify this, you can ask some provocative questions.
a) “Doctor, I’ve read that adults, too, need to be vaccinated, can I get myself vaccinated?” The doctor confirms that adults every 10 years should be vaccinated (diphtheria, tetanus) and will offer you options on how it can be done.
b) “Doctor, can you drink enzymes to aid digestion, I sometimes feel bad after I eat a lot?” The doctor will try to explain that the enzymes are not shown to “aid digestion” and advise you to be cautious in the diet.
b) “Doctor, I would like to drink a course of hepatoprotectors, can you give me to pick them up?” The doctor you try to explain that taking medications has proven effective.”
Doctors may not be enough
The health Ministry planned that doctors will be competing for the patient, since this will depend on their earnings. But the doctor is not a very good idea of what to do with the declarations.
“I don’t know how it is really possible to do what it is planned. We enclose the Declaration, and then what will happen? The money also will be allocated by population, will not be considered by districts and declarations. It’s the same, only in profile” – said the physician Alexander Ryltsov.
The government plans to pay in 2018 for each patient 370 UAH per year for children and pensioners – UAH 740, but the money will not the doctor and the hospital. If the doctor is not satisfied, it can be changed.
However, all may not be enough doctors. According to various estimates, in Ukraine on the primary level of medicine is not enough by 30% to 50% of specialists. Particularly acute problem in rural areas. Therefore, the Ukrainians may face a limited choice in signing the Declaration on HIV / AIDS care.
“Just yesterday, I communicated with family physicians. I can conclude that they don’t know how it will happen. Any reform needs to consider everyone from whom she would go.
